Background:Finasteride, a nonsteroidal inhibitor of prostaglandin synthesis, is a widely used agent in dermatology for the treatment of benign prostatic hypertrophy (BPH) and other diseases associated with prostate enlargement. The effect of finasteride on prostate-specific antigen (PSA) was evaluated in a randomized, double-blind, placebo-controlled study with 12 healthy men. The study was conducted at the Department of Dermatology, The University of Western Ontario, Ontario, Canada. A total of 12 patients were randomised to one of two treatments: finasteride (0.1 mg/day) and placebo (0.1 mg/day). In the first study, finasteride was administered at least weekly and at the same time in the morning, while in the second study the patients were treated with finasteride weekly for a mean duration of 12 weeks. In both studies finasteride significantly decreased PSA in a dose-dependent manner in both the placebo and finasteride groups. Finasteride significantly reduced the mean maximum urinary concentration of the PSA in the finasteride groups. The difference between finasteride and placebo in the mean maximum urinary concentration of PSA was not significantly different in the finasteride groups. The most common adverse events were urinary retention, ejaculation, and decreased libido. These results suggest that finasteride may have a greater effect on the prostate than the placebo in men with BPH.

The study design is based on a randomized, double-blind, placebo-controlled study in a group of 12 healthy men. Patients were randomly assigned in a 1:1 ratio to one of the following treatment: finasteride (0.1 mg/day) or placebo (0.1 mg/day). PSA levels were measured using a biochemistry-based technique. We assessed the effect of finasteride on the PSA and serum PSA in the two groups. The results showed that the mean maximum urinary concentration of the PSA was reduced in the finasteride group when compared to the placebo group. In addition, the finasteride group showed a statistically significant increase in the mean maximum urinary concentration of PSA when compared to the placebo group. However, the difference did not reach statistical significance. These findings suggest that finasteride may have a greater effect on the prostate than the placebo in men with BPH. We also observed that the effect of finasteride on serum PSA was not significantly different between the finasteride group and the placebo group. Finasteride has been shown to decrease the PSA in patients with BPH. Therefore, it is recommended to prescribe finasteride for patients with BPH and BPH-related prostatic hyperplasia as a first-line treatment.
